Capybara

Appearance: Blanc becomes a capybara.
Passive Effect: None.
Action: (Z) Blanc kneels on the ground.
Practical Uses: None.
Interact with the turquoise forest painting in the Nexus. In the snowy world, go north-west and enter the large building via the door on the right side. Go through the wooden door at the top of the locker area. Interact with the capybara walking around in the next room.
Jellyfish
Float on.
Appearance: Blanc wears a puffy dark blue dress and has an extra pair of legs.
Passive Effect: None.
Action: None.
Practical Uses: None.
(Doctor effect is needed.)
Use the Doctor effect until you get teleported to an underground sea area. If you teleport to a bench in an underwater cave, simply go south-east and interact with the pink and green jellyfish pair floating in an open area.
Alternatively, if you are teleported to an underwater area with a streetlamp to your right, go south-east and enter the cave. Inside the cave, go north and interact with the pink and green jellyfish pair floating in an open area.
Cat
Don't forget the flea collar.
Appearance: Blanc becomes a cat.
Passive Effect: Allows Blanc to fit through small spaces. Blanc will also be able to attack mice in the sewer area.
Action: (Z) Blanc makes a noise.
Practical Uses: None.
Interact with the building painting in the Nexus. Go left and up, hugging the wall you were standing in front of, and enter the door closest to your right. Interact with the cat sitting in the basketball court.
Doctor
An apple a day...
Appearance: Blanc is dressed as a doctor.
Passive Effect: None.
Action: (Z) Blanc is teleported to a random area in the dream world.
Practical Uses: Can be used to quickly reach certain areas.
Interact with the paper painting in the Nexus. Interact with the elevator on the rooftop to reach a waiting room, then go up the stairs to the right. Interact with the window on the stairway, then go up the stairs to the left. You should reach another waiting room. Interact with the clock on the wall, step away from the chair and wait until the door opens. Blanc will enter the room and you will be given the effect after they exit.
Creature
You can't change this...
Appearance: Blanc becomes a shadowy creature.
Passive Effect: None.
Action: None.
Practical Uses: None.
(Cat effect is needed.)
Interact with the paper painting in the Nexus. Interact with the elevator on the roof. In the waiting room, go down then go left past the hospital beds to reach another waiting room. Enter the door then stand over the toilet in the open bathroom stall. In the sewer area, go left, take the south path at the crossroads, then take the first left and enter the doorway.
In the next area, go south until you hit a wall. Go right until you are stopped by the edge, then go up. At the first right path you should see a hole in the wall. Equip the Cat effect and walk into the hole. Go south and walk into the small hole at the dead-end. Enter the doorway on the other side then interact with the fishing creature.
OR
Interact with the building painting in the Nexus. Go left to the wall, then go up and left again. Enter the door with the gray sign on it then step over the toilet in the open stall. In the sewer area, go left then walk the doorway to your bottom-left. Go south from the doorway until you hit a wall. Go right until you are stopped by the edge, then go up. At the first right path you should see a hole in the wall. Equip the Cat effect and walk into the hole. Go south and walk into the small hole at the dead-end. Enter the doorway on the other side then interact with the fishing creature.
Skates
For the girl on the go.
Appearance: Blanc wears roller skates.
Passive Effect: Blanc's movement speed increases.
Action: None.
Practical Uses: Can be used to navigate the dream world faster.
The skates have a random chance of spawning in the dream bedroom.

Use the Doctor effect until you get teleported to an underground sea area. If you teleport to a bench in an underwater cave, go north-east and enter the cave for a cutscene.
If you get teleported to an underwater area with a streetlamp to your right, go south-east and enter the cave. Inside the cave, go down then enter the cave to your right.
